Ever since men and women started losing hair, there have been 'remedies' to stop or reverse the process, and many a con-artist or trickster happy to make a quick buck through the misery and misfortune of others. I can imagine one of our distant ancestors buying a toupee fashioned out of twigs only to be ridiculed by the rest of the tribe.

Indeed potential cures have ranged from Wiccan magic spells to getting various animals, commonly cows in India to lick the scalp of a balding man. An eighth century Chinese remedy which involves catching a green grass snake and pounding it to death in a bag of silk and then boiling the remains and rubbing them into your scalp and the Native Americans had a similar remedy, replacing the snakes with scorpions, nettles and the urine from a pregnant woman!

On a more serious note, bearing in mind all causes of hair loss as discussed in my other articles, there are only two types of treatments are going to work, those which address the hormonal sensitivity, or treatments that stimulate growth despite it. And as it happens there are only two drug treatments approved by the U.S. Food and Drug Administration (FDA) for male baldness: Minoxidil and Finasteride. And we will begin by looking at Minoxidil today.

Minoxidil (more commonly known by its trade names Rogaine and Regaine) is a vasodilator medication which was first used exclusively as an oral drug to treat high blood pressure. However those using it will noticed an interesting side effect, namely increased growth and darkening of fine body hairs.

Seeing the potential of this Upjohn Corporation produced a topical solution that contained 2% minoxidil to be used to treat baldness and hair loss, under the brand name Rogaine in the United States and Canada, and Regaine in Europe and the Asia-Pacific. Treatments are available in 5% concentration solution for men and 2% concentration solutions for women.

How Minoxidil works is still not fully understood but studies have shown it is effective in preventing hair loss and may help attain some regrowth. One particular study in healthy males aged 18-50 years with male pattern baldness found that compared to a baseline of 103 to 106 hairs/cm2, those who applied of a 5% solution of minoxidil for 32 weeks increased their hair counts by an average of 39 hairs/cm2, in contrast to 5 hairs/cm2 in subjects who received a placebo.

It should be noted that Minoxidil has shown to be more effective in younger men (18 to 41 years of age) and has shown to work only in the central vertex are, so will not help those with a receding hairline.

As with most medicines some side affects should normally be expected however Minoxidil fares pretty well in this department.

The most common side effect of topical (i.e. external use) is in fact an itchy scalp, dandruff and hair loss. Yes you did read that correctly, hair loss is a common side effect of minoxidil treatment, however manufacturers and users say this is part of the shedding and regrowing effect. However there is no guarantee that the new hair loss will be replaced with hair growth. Regardless Minoxidil remains one of the most widely used treatment of balding in the world today. It is my personal recommendation for slowing down the balding process before you consider hair transplant.

First of all, don't panic! Hair loss or hair shedding is consistent within the hair growth cycle and it is normal to lose some scalp hair each day. The average human scalp has roughly 100,000 to 150,000 individual hairs and the normal hair growth cycle results in the loosening or shedding of about 100 to 150 hairs on a daily basis. New hair growth then emerges from these same previous dormant hair follicles, growing at the average rate of about half an inch per month.

Hair is composed of two separate parts: the follicle and the hair shaft. The follicle lies below the scalp and produces the hair strands that we see growing out of our head. The follicle is alive, however the hair strand is simply composed of dead cells that have no regenerative ability.

When the dormant period ends the hair is shed; these are the worrisome hairs we obsess over in our comb, hairbrush, on our pillow or down the shower drain. Relax, some hair loss is perfectly normal.

Baldness or Alopecia happens when the normal pattern of hair growth is disrupted. The normal pattern of human hair growth is growing, resting, shedding and growing again. If the growth pattern is out of balance, hair does not grow back as readily as it falls out. A family history of androgenetic alopecia increases your risk of balding. Heredity also affects the age at which you begin to lose hair and the development, pattern and extent of your baldness.

What concerns us is not these normally shed hairs, but the noticeable thinning we confront in the mirror. For a woman, thick, vibrant hair is our crowning glory, our vanity visible. Baldness is not usually caused by disease, but is more commonly related to heredity, aging and hormone function. This androgen can be altered by enzymes and turned into testosterone. When females have this problem DHEA precedes DHT. It is converted by alpha 5 reductase enzymes and hydroxysteroid hydrogenase isomerase, which is an enzyme. Hair loss can be caused by a variety of factors with heredity being the number one reason why men and women lose their mane. Hereditary hair loss in men and women happens to have the same main cause, which is dihydrotestosterone (DHT) attacking our hair follicles. DHT is a metabolite of the male hormone testosterone but it is also present in the female body. Men and women lose their hair not because of increased levels of DHT in their bodies but due to the susceptibility of their hair follicles to harmful DHT attacks, although the exact mechanism and reasons why certain hairs, mostly those on the top of the head, are more susceptible to such attacks are not yet know.

The main difference between the male and female form of hereditary hair loss seems to be in its shape. The male form has a characteristic horseshoe pattern whereas the female form is typically a diffuse thinning across the entire scalp and is thus less easily-recognised. The female form of hereditary baldness is, in its form, practically indistinguishable from hair loss caused by various other factors. The second most common reason for a woman to lose her hair is hormonal imbalances during and after pregnancy or menopause. Such changes are typically of a temporary nature and so also is the hair loss, although post-menopausal hair loss is in most instances permanent.

When it comes to treating hair loss in female patients there are a number of specifics. First, most women do not make very good candidates for hair transplant surgery due to their diffuse thinning pattern, which makes it impossible to identify the hair that will be resistant to future miniaturisation. Secondly, finasteride (Propecia), which is one of the only two FDA-approved hair loss treatments available today, cannot be used on women and neither can dutasteride, which is its closest and assumedly yet more powerful relative. Rogaine (generic name minoxidil) is the second FDA-approved hair loss treatment and its approved concentration for women is only 2% versus 5% for men. However, many doctors recommend their female patients use male formulations of minoxidil, with a concentration of 5%, such as Rogaine foam. Aminexil is a molecule similar to minoxidil and it is often recommended to women who are pregnant or breastfeeding as a safer option than minoxidil.

Although some anti-androgens such as finasteride or dutasteride cannot be prescribed to women, there are other anti-androgen medications that can be effectively used to treat female hair loss. Spironolactone (trade name Aldactone) and cyproterone acetate (used in contraceptives such as Diane 35, Diane 50 and Ginette 35) are anti-androgens most commonly used to treat female pattern baldness.

Another treatment with seemingly satisfactory rates of success is topical estrogen solutions such as Crinohermal, which use a female hormone, estradiol, as their main active substance. Estradiol is capable of inhibiting the conversion of testosterone into follicle-harming DHT. Hormone replacement therapy is another hormonal treatment suitable for women at menopause, with estrogens and progesterone pills and creams being the most common forms of treatment. Estrogen levels in the body decline with age. As women enter the menopause, estrogen levels decline and more of the male hormone testosterone is then available to be converted into follicle-harming DHT.

This list of treatments for female hair loss is not exhaustive, though. There are a number of other remedies that are often claimed to help promote hair growth in women, such as ketoconazole, flutamide, fluridil, alfatradiol, as well as various substances of natural origin. Despite the fact that there are female patients who will swear by some of these treatments, none of them has ever been sufficiently clinically tested, let alone approved by any major national health supervisory authority as a treatment for female baldness and the claims of their guaranteed effectiveness should be taken with a grain of salt.

While it's common to see and hear about men's hair loss, women's hair loss is seldom mentioned. It's almost as if society doesn't want to admit there's such a condition. The purpose of this news letter is to define the problem and its causes; as well as to explain some of the solutions that are available.

What is excessive hair loss?

All people, men and women lose hair and it's natural to do so. A normal person loses from 15 to 40 hairs every day.

Hair goes through three growth cycles. The Anagen phase is when hair grows. This cycle lasts about three years. The second phase is called the Catagen phase. During this time hair growth ceases and no pigment is produced for about 10-days. The Catagen phase is followed by the Tologen phase in which the hair is shed. This cycle usually lasts for about three months.

In individuals with healthy follicles about 90% of the hair on the scalp grows at one time. Because the vast majority of follicles are in the Anagen phase- and only a small percentage of them are in the Tologen phase-a normal amount of hair fall out isn't noticeable.

The overwhelming majority of women suffer from what is known as Androgenic hair loss; this is caused by hormones.

When a follicle is unhealthy for whatever reason, the hair growth cycles are suspended and the follicle stays in the Tologen phase for an indefinite period. As more and more follicles stay in the Tologen phase less new hair is being produced. This results in thinning and ultimately, balding!

How long do afflicted follicles stay in the Tologen stage? In many cases forever; in some cases hair production resumes. The determining factor is what caused the follicle to stop producing hair in the first place.

Why women lose hair?

Women can lose hair for a variety of reasons. In pregnancy changes in hormone levels can produce hair loss. Stress and anxiety can also cause hair follicles to cease production. In some cases a vitamin deficiency can lead to hair loss. Certain medications can cause hair to fall out too. But in all the aforementioned cases, the hair loss is only temporary for most people. Once the condition causing the hair loss ceases either naturally or through intervention, the hair follicles will "wake up" and begin producing hair once again.

There are two conditions in which hair loss is irreversible. The first is from the condition known as alopecia universalis. Very few people are afflicted with alopecia universalis, but those who are facing the devastating effects of all hair production ceasing on their body-they actually produce no hair at all, from their scalps to their toes. The condition is believed to be caused by a virus and there is no "cure" for it.

The overwhelming majority of women suffer from what is known as androgenic hair loss. This is caused by hormones. Both men and women have hormones of the opposite sex. Men have levels of estrogen in their body, just as women have levels of testosterone. These women are just unable to "break down" testosterone properly.

There is much testosterone found in a person's scalp. When too much DHT accumulates in the scalp, hair follicles are affected. They begin to atrophy. The hair being produced becomes smaller, weaker in structure, and lighter in color. Finally, the hair follicle enters a permanent dormant state and no hair is produced at all. In most cases, there is no way to induce the hair follicle to produce normal, healthy hair again. The hair follicle is essentially dead.

What can be done?

In cases of hair loss that are due to stress, medication or pregnancy, hair growth will return to normal as soon as the condition causing the hair loss ceases to exist. When hair loss is caused by scalp disorders or vitamin deficiencies, these conditions can be corrected with the proper therapies, many of which can be obtained without a prescription.

However, for the vast majority of women suffering from female patterns baldness (FPB), the answers and corresponding solutions are not as easy to come by.

Topical Lotions

There are many topical lotions sold by beauty salons across the country, specifically for women with thinning hair. For many women, this is the fist attempt at correcting their excessive hair loss. However, for women with androgenic hair loss (female patterns baldness), lotions and creams simply will not work.

Drug Therapy

Minixidil has now been approved for use by women in formulations containing 2% of the drug. Now sold as an over-the-counter product, minoxidil has been approved by the FDA as a hair loss cessation/ hair growing drug. However, minoxidil has only been shown to grow hair in the crown, not in the frontal hairline. The hair that minoxidil can grow, even in the crown, is usually hair that is not considered "cosmetically acceptable", meaning hair that will not grow long and healthy enough to cover the scalp.

As for minoxidil's ability to stop hair loss, the success rate varies widely from individual to individual. In most cases, the hair loss still continues but will sometimes do so at a lesser rate. In all cases, once minoxidil use is stopped, hair loss returns to its original levels; any hair growth achieved will also cease.

Transplants

Transplants are now being performed by doctors on women patients. Hair transplants have been vastly improved in the past ten years, and no longer produce the "row of corn" appearance that was the case in the days of "hair plugs".

It must be noted that transplants do not create new hair. They simply move hair from the back of the scalp (the donor area) to the desired areas of the scalp where there is hair loss. The amount of hair on the scalp itself remains the same. It is just rearranged. In order for transplants to be successful, the patient must have enough hair in the donor area to cover the thin or bald areas. If enough donor hair can't be harvested, significant cosmetic coverage will not be achieved in thin or bald areas. Another consideration is future hair loss: since your natural hair continues to fall out, the question then becomes, will there be enough donor hair to eventually cover the balding areas without leaving the donor area denude of hair too?

The reason that transplants are more widely touted for men and not women has to do with the amount of hair that can be harvested. Both men and women have the same number of hairs on their scalps, but in men, it is more acceptable for light coverage to be the result-it's better for a man to be thinning than bald, is the theory. For women, thin hair that makes her look like she is going bald is rarely acceptable.

Wigs

Standard machine-made wigs offer a number of advantages. They provide full coverage of the thinning areas, for one. And unlike many so-called women's hair loss solutions, they're guaranteed to work.

But machine-made wigs are not for everyone. The less expensive ones that use synthetic hair can be less than natural looking. Machine-made wigs can also be hot and cumbersome. For women with an active lifestyle-especially women who exercise and engage in other physical activities-wigs can be limiting: you certainly can't swim in them, and even sunning or being intimate can be compromised by a wig's limitations. And of course, they're not part of you; at night, they generally come off. For some women, the advantages far outweigh the disadvantages. For other women, wigs are not a viable solution. It depends on lifestyle, your expectations, and the trade-offs you're willing to accept. In other words, whether or not to wear a wig is an intensely personal decision.
